Chronic Condition Management Plans and Health Assessments
What is a Chronic Condition Management Plan?
A Chronic Condition Management Plan (CCMP), also known as a GP Chronic Condition Management Plan (GPCCMP), is a personalised care plan developed by you and your GP to help manage long-term health conditions.
From 1 July 2025, the CCMP replaced older plans such as the Chronic Disease Management Plan (CDMP) and Team Care Arrangements (TCA).
Who is eligible for a CCMP?
You may be eligible if:
- You have a chronic or terminal condition that has lasted, or is expected to last, six months or more
- Your GP believes you would benefit from a structured care plan
- You are registered with a MyMedicare practice, or regularly see your usual GP
Allied Health Support
A CCMP can provide access to Medicare-subsidised allied health services, including:
- Up to 5 sessions per calendar year
- Up to 10 sessions per year for Aboriginal and Torres Strait Islander patients
These services may include physiotherapy, dietetics, podiatry, psychology, exercise physiology, occupational therapy, and group services for conditions such as type 2 diabetes, where appropriate and aligned with your care plan.
How to Get a CCMP
- Book an appointment with your GP and let them know you would like a CCMP. A follow-up appointment with a nurse may be arranged to help complete your plan.
- Your GP will assess your condition(s), work with you to set goals, and develop your personalised care plan.
- Once completed, your GP will provide you with your plan and any necessary referrals.
- You can then attend your approved allied health services. If you wish to change providers, you can contact the clinic to arrange an update to your referrals.
- Regular reviews with your GP are recommended to monitor your progress and update your plan as needed.
Important Information
- Unless otherwise specified by your GP, referrals to allied health services are valid for 18 months.
- If you had an existing GP Management Plan (GPMP) or Team Care Arrangement (TCA) before July 2025, these will remain valid until 30 June 2027.
